With a head office in Llandrindod Wells, Cymryd Rhan has offices and supports vulnerable people across Mid and North Wales. 

  • We believe that all people have the right to be as independent as they can.
  • We believe that all people can be active in the community.
  • We believe in looking at what people can do instead of what they can’t.

That’s why we are called Cymryd Rhan, which means taking part. And that’s why our site talks to the people we support.

 

WHO CYMRYD RHAN SUPPORT

If you are:

· A person with a learning difficulty or a physical disability.
· An older person.
· Or a person with mental ill health.
 
 

We can help you live an active, independent life by giving you whatever support you need.

 

 

WHERE CYMRYD RHAN SUPPORT

 

We can support you:

  • In your own home.

  • In a shared house.

  • At work.

We help people like you right across Mid and North Wales.

 

 

 

HOW CYMRYD RHAN SUPPORT YOU

We don’t say we know what’s best for you and we don’t try to tell you the right way to do things. We encourage you to do as much as possible for yourself.

We believe that you’ll do best when the support you receive is designed to suit you. We call this person-centred support.

This means that the way we support you will be centred around you because everybody’s needs are different.

 We’d like to:

  • Match you with well-trained and qualified support workers to give you on-going support.

  • Support you in any way to do the things you want to do.

  • Make sure that the support is there only when you want it.

CHOOSING WHICH SUPPORT

You choose the support you receive. We can help you with:

  • Health and personal care.

  • Managing your house.

  • Household tasks.

  • Safety in the home.

  • Managing money and communicating, including bills and letters.

  • Playing sports and ways to relax.

  • Jobs and being a volunteer.

  • Mixing with people and making friends.

  • Night sitting and sleep-in staff.

 

 

HOW SUPPORT WORKERS HELP

Once you have picked a support worker you will then work together to plan exactly how you want each day to go. 

Your support worker will write down what you want and put it into a support plan, called an Essential Lifestyle Plan, so that they can support you in the way that you want.

One person’s experience:
When Mr X’s mother passed away in 2004 he was in his forties and didn’t know how to live alone. We supported him as he moved from shared living with 24-hour support into his own flat. Over time we helped him to develop new skills and become part of the community, so that he needed us less and less. Mr X now lives on his own, with no need for services or care. He says:
“My life is how I want it to be”.
